一、本人向学校提交申请，获得推荐资格后，填写《20**圆梦行动助学金申请表》（一式四份），经所在学校初审并加盖公章后报所在县团委(其中高考成绩，录取院校,院、系，专业为空白。所有申请资助学生，在当地工商银行或农业银行开设银行卡的`将卡号及详细开户行名称（如中国农业银行山东省××市分行××县支行××分理处）填写完整。
三、当你接到全日制普通高等院校录取通知书后，应在三日内到所在县团委补填《20**圆梦行动助学金申请表》中的高考成绩，录取院校，院、系，专业等栏目信息。
四、市希望办将利用网络等媒体登载所有申请资助人《20**圆梦行动助学金申请表》中的相关内容，信息不全者不公布。
五、市希望办将组织志愿者对申请资助同学进行家访，经资料审核确认后，将根据报送录取生高考考分、录取院校、考生综合素质以及本区县圆梦行动开展情况等因素择优选择资助对象。
六、你的受助情况我们将在公益0533网站及时更新披露，你可以通过该网站了解你的资助情况。如果你获得捐款人的资助，我们会通过团县委通知你，请你参加助学金发放仪式。
